Because the applicant cannot be considered for acceptance without this recommendation, it is crucial that we receive your evaluation in a timely fashion. - Pastoral: This recommendation should be completed by a pastor or other full-time ministry staff at the church the applicant now attends. If that person is a relative, an elder or other church officer may substitute.
- Mentor or Discipler: This recommendation should be completed by an individual who has been discipling / mentoring the applicant.
- Teacher or Employer: This recommendation should be completed by a teacher or academic advisor or by an employer or work supervisor. A teacher or employer who is a family member should not complete this recommendation.
- Personal: This recommendation should be completed by a friend who has known the applicant for no less than one year, is not a family member, and is able to evaluate the applicant on the basis of the characteristics listed on this form.
